What Has Been Documented

Independent investigators and oversight bodies have repeatedly focused on California programs, including:

  • Hospice and home-health billing — Facilities and networks accused of enrolling patients who do not meet criteria or of billing for services not rendered
  • Medi-Cal and Medicaid-related schemes — High-dollar claims patterns that have drawn state and federal scrutiny
  • Daycare and childcare subsidy issues — Cases overlapping with broader national patterns of alleged empty or low-attendance facilities still receiving payments
  • Government waste and audit gaps — Reporting on delayed or incomplete audits and official acknowledgments that oversight is lagging

Much of the most widely circulated footage has come from on-the-ground citizen journalists who visit addresses, record what they find, and publish the primary video. Separate official actions (raids, charges, legislative responses) have followed some of these public investigations.

Why California Matters

Scale. California’s programs are large enough that even a small percentage of improper payments represents enormous sums.
