Tell Your Story

This record exists because people who lived it decided to say it out loud. If the VA failed you, if a claim died in a drawer, if you watched fraud happen from inside the system, or if the medical examiner’s office failed your family — this is the door. Tell it once, here, and it lands with the whole network: DearDougCollins.com (the accountability record), BrownWaterVet.com (the veterans’ resource hub), and AllegedFraud.com’s VA Fraud Case Files.

You set the terms. Every permission below is yours to give or withhold, and nothing is published that you did not permit in writing.

Did the medical examiner fail your family?

Did the medical examiner fail your family? A medical examiner who declined jurisdiction, sat on a case, refused or botched an autopsy, lost tissue or evidence, or stonewalled a family has failed twice — once at the table, and once at the record. If that happened to you, choose “The medical examiner failed us” in the form below and tell us what happened. One story is an anecdote; a pattern of stories is a case — and patterns are what this network documents.

For whistleblowers — read this first

Do not submit from a work computer or a work network. Do not include classified material or any patient’s identifying information.

No name is required. If you work — or worked — inside the system and you saw fraud, waste, or abuse, you do not have to carry it alone, and you do not have to go unprotected. Before or alongside telling us, know your protection routes: the Government Accountability Project and the National Veterans Legal Services Program (NVLSP).

Whistleblower tips are never published raw. They go to verification and to the legal-protection route first.

Contact is optional. Anonymous submissions are accepted.

Privacy notice. Every submission is read by a person and routed by category. Your story is published only on the written permission you gave above — with your name, anonymously, or not at all. If you checked “record only,” that overrides any publish permission, and your story stays in the record, unpublished. We do not sell or share your contact details.

If you have documents you want to share, reply to our acknowledgment email and we’ll send you a secure way to send them — never email documents with Social Security numbers.

We are not a law firm and this is not legal advice.
